Building MindEase: My Journey Creating an AI Therapist That Actually Cares

How I built a real-time mental health chatbot that responds in milliseconds, detects crises, and talks like a friend—not a robot


When I started building MindEase, I had one clear goal: make mental health support as accessible as sending a text message. No appointments, no judgments, no barriers. Just open the app and talk.

But here's the thing about building a mental health chatbot—it's not just about connecting an AI to a chat interface. It's about creating something that feels human, responds instantly, knows when someone's in crisis, and never makes them feel like they're talking to a machine.

This is the story of how I built MindEase, the decisions I made, the problems I solved, and the lessons I learned along the way.

Why I Built This

Let me start with the problem. In Nigeria, where I'm from, mental health support is practically inaccessible for most people. There's stigma, there's cost, and honestly, there just aren't enough therapists. Even in places with better healthcare, therapy appointments are weeks away, and crises don't wait.

I kept thinking: what if someone needs to talk at 2 AM? What if they're too scared to call a hotline? What if they just can't afford ₦15,000 per session?

That's when I decided to build something different. Not a replacement for therapy—I want to be clear about that—but a bridge. A first step. A friend you can talk to when there's no one else.

The Vision: More Than Just a Chatbot

From day one, I knew this couldn't be a typical chatbot. You know the ones—they give you canned responses, don't remember what you said three messages ago, and feel robotic. That wasn't going to work for mental health.

I needed MindEase to:

  • Respond like a human friend, not a clinical textbook
  • Remember our conversation, keeping context across messages
  • Detect when someone's in crisis and provide immediate resources
  • Feel instant—because waiting 5 seconds for a response breaks the flow
  • Protect privacy completely—anonymous by default, no personal data required
  • Be culturally aware—understand Nigerian expressions and communication style

But most importantly, it had to feel safe. Like talking to that friend who just gets it.

  • 24/7 Availability: Always accessible, no appointments needed
  • Complete Anonymity: Users can seek help without revealing their identity
  • Instant Support: Immediate responses to emotional distress
  • Cost-Free Access: No financial barriers to entry

Project Vision and Objectives

To create a compassionate AI companion that makes mental health support universally accessible, reducing barriers to emotional wellness through technology.

The Architecture: Thinking Through the Design

Before writing a single line of code, I spent days just thinking about how this should work. The architecture needed to solve some very specific problems:

Problem 1: Speed
If someone types "I'm feeling really anxious," they shouldn't wait 5 seconds for a response. That's an eternity when you're anxious. The conversation needs to flow naturally.

Problem 2: Memory
The AI needs to remember what we talked about. If you tell me you're stressed about exams, and three messages later I ask "what's bothering you?"—that's failure.

Problem 3: Realness
This can't feel like talking to a bot. It has to feel like FaceTime with a friend who's really listening.

Problem 4: Safety
If someone types words like "suicide" or "hurt myself," I need to know immediately and provide help—actual help, not just "sorry to hear that."

The Solution: Real-time Everything

I decided on a three-layer architecture, but more importantly, I made real-time communication the foundation. Here's why:

Traditional REST APIs work like this: You send a question, wait, get an answer. That waiting kills the conversation flow. Instead, I used WebSockets—a persistent connection between the user and server. Think of it like leaving a phone line open instead of hanging up and calling back every time.

This choice cascaded into everything else. With WebSockets, I could:

  • Send AI responses word-by-word as they're generated (streaming)
  • Show typing indicators in real-time
  • Detect crises and show resources instantly
  • Keep the conversation feeling alive

The flow looks like this:

You type → WebSocket sends it instantly → My backend receives it
    ↓
Backend combines your message with conversation history
    ↓
Sends to OpenAI GPT-3.5 Turbo with therapeutic instructions
    ↓
AI starts generating → Each word comes back → Backend forwards it
    ↓
You see words appearing one by one (like someone typing)

It's the same flow as ChatGPT, but optimized for emotional support.

Building the Backend: The Decision Points

Choosing the Stack

For the backend, I needed something that could handle real-time connections efficiently. Node.js was the obvious choice—it's built for this. The event-driven, non-blocking architecture means it can handle thousands of WebSocket connections without breaking a sweat.

I paired it with TypeScript because when you're building something people depend on for mental health, you can't afford silly bugs. TypeScript catches them before they reach production.

For the database, I went with PostgreSQL + Prisma. Here's why that matters:

PostgreSQL gave me ACID compliance (consistency guarantees), and Prisma gave me type-safe database queries. The developer experience is incredible—autocomplete for database fields, compile-time error checking, and automatic migrations.

Designing for Anonymity First

One of my core principles was: you shouldn't need to give me your email to talk to me. So I built the system to support anonymous users from day one.

When you first open MindEase, I create a user account for you automatically with just a unique hash. No email, no password, no nothing. You can start talking immediately.

Want to save your conversations across devices? Then you can add an email later. But that's optional—always.

The database reflects this philosophy. The email field is nullable, isAnonymous defaults to true, and every user gets a hashedId whether they register or not. Privacy by design.

The Crisis Detection System: This Took Real Thinking

This was probably the hardest part to get right. If someone says "I want to end it all," I need to:

  1. Recognize it immediately
  2. Store it securely for monitoring
  3. Provide real resources—not just "please call someone"
  4. Do this without being intrusive for normal conversations

I built a two-layer detection system:

Layer 1: Keyword Scanning
Before the message even reaches the AI, I scan it for crisis keywords. I organized them by severity:

  • CRITICAL: "suicide", "kill myself", "end my life"
  • HIGH: "self-harm", "hurt myself", "can't go on"
  • MEDIUM: "hopeless", "worthless", "give up"
  • LOW: "depressed", "anxious", "overwhelmed"

Layer 2: AI Analysis
Sometimes distress is subtle. "I'm fine, just tired all the time" might not trigger keywords, but context matters. So for borderline cases, I could send the message to a separate AI call for emotional analysis (this feature is planned but not yet implemented—keyword matching proved surprisingly effective).

When a crisis is detected, a CrisisAlert gets saved to the database with:

  • The exact message that triggered it
  • Severity level
  • Conversation context
  • Timestamp for monitoring

Then the frontend shows a modal with actual, callable numbers. Not generic advice—real, Nigerian mental health resources:

This isn't just for show. If MindEase is going to exist, it has to genuinely help in crisis moments.

The Frontend: Making It Feel Human

The UI Philosophy

I wanted the interface to feel like WhatsApp or iMessage—familiar, comfortable, not intimidating. If it looks like a medical app, people won't open up.

I chose React with Tailwind CSS and shadcn/ui components. Tailwind let me build quickly without writing CSS files, and shadcn gave me beautiful, accessible components that I could customize.

But more than the tech stack, I focused on the feel. Every design decision was tested against one question: "Would someone having a panic attack at 2 AM find this easy to use?"

The Chat Experience: Streaming is Everything

The most important UX decision I made: streaming responses.

When the AI generates a response, I don't wait for the full response. I show you each word as it's generated. This creates the feeling of someone typing—and it's not just a feeling. The AI is "thinking" and "typing" in real-time.

Here's the user experience:

  1. You send a message
  2. A typing indicator appears immediately
  3. First words show up after ~500ms
  4. More words stream in, one chunk at a time
  5. Full response appears in 2-4 seconds total

Compare this to waiting 4 seconds seeing nothing, then BOOM, full response appears. That feels robotic. Streaming feels human—like you're talking to someone who's thinking about what to say.

Implementation-wise, I'm using Socket.IO to send chunks:

The result? It feels like you're texting a friend, not querying a database.

Mobile-First, Always Responsive

Most people in Nigeria access the internet primarily through phones. So I designed mobile-first, then scaled up for tablets and desktops.

On mobile: Single column, full-screen chat, bottom navigation
On desktop: Sidebar with conversation history + main chat area

I tested it on iPhone SE (the smallest screen common today), and it had to work perfectly there first. If it works on iPhone SE, it works everywhere.

The AI: Teaching a Machine to Be Compassionate

This is where things get interesting. The quality of MindEase lives or dies on how the AI responds. I could have the fastest backend and prettiest UI, but if the AI sounds like a customer service bot, nobody's coming back.

Why GPT-3.5 Turbo?

I chose OpenAI's GPT-3.5 Turbo for a few pragmatic reasons:

  1. Cost: At ~$0.002 per conversation, it's sustainable. GPT-4 would be 10-15x more expensive.
  2. Speed: Response latency of 500-800ms to first token. GPT-4 is noticeably slower.
  3. Quality: For therapeutic conversation, GPT-3.5 Turbo is more than sufficient—the prompt matters more than the model.
  4. Availability: No waitlists, no API quota issues, reliable uptime.

Prompt Engineering: The Real Secret Sauce

Here's the truth about AI chatbots: the model is less important than the prompt. You can build a terrible therapist with GPT-4 and a great one with GPT-3.5 Turbo—it's all about how you instruct it.

I spent weeks refining the system prompt. Here's my thinking process:

Principle 1: Sound Human, Not Clinical

I explicitly tell the AI to avoid psychology jargon. No "cognitive distortions," no "maladaptive coping mechanisms." Instead, talk like a friend:

"You are a compassionate AI emotional support companion. Your role is to help users feel heard, grounded, and supported while gently guiding insight using evidence-based therapeutic principles (CBT Principles) without sounding clinical."

Principle 2: Cultural Attunement

Nigerian communication style is warm, direct, sometimes uses Pidgin English. The AI needed to match that energy:

"Validate emotions with culturally familiar warmth: 'I hear you.' 'You did well to talk about this.' 'That one is not easy at all.'"

This was crucial. If someone types in Pidgin ("I no fit cope again"), and the AI responds in formal English, it feels alien. The AI needs to code-switch naturally.

Principle 3: Variety in Empathy

One problem I noticed in testing: the AI would start every response with "I hear you" or "That sounds really difficult." After three messages, it's like, "Okay, I get it, you hear me."

So I added this rule:

"Validation Variety Rule: Do NOT use the same emotional validation phrasing repeatedly. Vary how empathy is expressed:

  • Sometimes reflect the feeling.
  • Sometimes acknowledge the situation.
  • Sometimes start with a thoughtful question.
  • Sometimes begin with calm reassurance."

This one change made conversations feel 10x more natural.

Principle 4: Therapeutic Technique Without Naming It

I use CBT-inspired approaches, but I never say "this is cognitive reframing." Instead, I guide exploration:

"Ask reflective questions, not leading ones:

  • 'What was going through your mind at that moment?'
  • 'Is that the only way this situation can be seen?'
  • 'What makes this thought feel true right now?'"

The user doesn't need to know they're doing CBT. They just need to feel heard and gently guided toward insight.

Context Management: Remembering the Conversation

GPT models are stateless—they don't remember anything. Every API call is a fresh start. To create continuity, I send the recent conversation history with each message.

Here's the strategy:

  • Store all messages in PostgreSQL
  • When a new message arrives, fetch the last 8 messages
  • Build a message array: [system prompt, ...last 8 messages, new user message]
  • Send to OpenAI

Why 8 messages? Trial and error. Less than 6, and the AI forgets important context. More than 10, and I'm paying for tokens I don't need. 8 is the sweet spot—about 4 exchanges.

This means if you told me you're stressed about exams, and three messages later you say "I'm still worried," I remember you're talking about exams. That continuity is everything.

The Streaming Flow

When you send a message, here's what happens in the backend:

The stream: true parameter changes everything. Instead of waiting for OpenAI to finish processing, I get chunks as they're generated. I forward them immediately via WebSocket, and you see words appearing in real-time.

Crisis Boundaries in the Prompt

The AI prompt includes explicit instructions for crisis situations:

"IF USER IS IN CRISIS (mentions self-harm, suicide, or severe distress):

  1. Respond with calm, grounded presence
  2. Validate their pain without minimizing
  3. Offer specific, local resources (Nigeria-based mental health services)
  4. Encourage reaching out to emergency contacts
  5. Do NOT attempt to 'solve' the crisis via chat"

This sets clear boundaries. MindEase is emotional support, not crisis intervention. The AI acknowledges the distress, provides resources, and encourages professional help—that's it.

Performance: Making It Feel Instant

Speed matters immensely for emotional conversations. If there's a 3-second delay after you pour your heart out, it kills the moment.

Here's how I optimized for speed:

WebSocket Connections

Unlike HTTP where every request establishes a new connection, WebSockets maintain a persistent connection. Think of it like leaving a phone line open. When you send a message, it goes instantly—no handshake, no overhead.

Connection time: ~10-30ms

Database Query Optimization

I use Prisma's selective loading to only fetch what I need:

Query time: ~5-20ms

AI Streaming

As I mentioned, streaming is the killer feature. Time to first token: ~400-700ms. That's when you start seeing the response. Full response might take 2-4 seconds, but you're not waiting—you're reading.

Frontend Rendering

I use React.memo to prevent unnecessary re-renders:

This means when a new chunk arrives, only the streaming message updates—existing messages don't re-render.

The Result

End-to-end latency (from your keypress to first AI word):

  • WebSocket send: 10-30ms
  • Backend processing + DB query: 20-50ms
  • OpenAI time to first token: 400-700ms
  • WebSocket return: 10-30ms

Total: ~500-800ms to first word.

That's fast enough to feel conversational. Not instant, but not awkward either—like texting someone who types quickly.

Security and Privacy: Non-Negotiable Principles

When people share their mental health struggles, they're trusting you with their vulnerability. I take that seriously.

Anonymous by Default

I said this before, but it bears repeating: you don't need to give me your email. The app works 100% anonymously. When you open it, I generate a unique hashed ID, create a session, and that's it.

If you want to sign up later (to access conversations across devices), you can. But it's optional, always.

JWT Authentication

For users who do register, I use JSON Web Tokens (JWT) for authentication. When you log in, I generate a token that expires in 7 days. Your browser stores it, and every request includes it.

The token contains: user ID, session ID, expiration time. Nothing sensitive.

Soft Deletes (GDPR Compliance)

If you want to delete your account, I don't actually delete your data immediately—I mark it as deleted (soft delete). This is for system integrity (foreign key constraints). But your email gets nullified, your hash gets replaced with "DELETED", and your conversations become inactive.

After 30 days, a cleanup job permanently deletes soft-deleted accounts. This gives users a grace period if they change their minds.

Security Headers

I use Helmet.js to set secure HTTP headers:

Rate Limiting

To prevent abuse, I limit each IP to 100 requests per 15 minutes. This stops spam attacks without affecting normal users.

The Challenges I Faced

Challenge 1: WebSocket Authentication

Socket.IO connections don't have traditional HTTP headers, so passing auth tokens was tricky. I solved it by sending the token in the connection handshake:

Challenge 2: Streaming Response Rendering

React's default behavior would re-render the entire message list every time a chunk arrived—60+ times per response. This caused jank.

Solution: Use React.memo and only update the last message:

Challenge 3: Crisis Detection False Positives

Early on, the keyword matching was too aggressive. Someone saying "I'm dead tired" would trigger a crisis alert. Not good.

I solved this by:

  1. Using phrase matching, not single words ("kill myself" not just "kill")
  2. Severity levels (LOW alerts don't show modals, only HIGH/CRITICAL do)
  3. Contextual checks (future improvement: use AI to validate keyword matches)

Challenge 4: Cost Management

OpenAI charges per token (input + output). Early iterations sent the entire conversation history every time—expensive and unnecessary.

Solution: Limit context to 8 messages, set max_completion_tokens: 1500, and monitor usage in the database. Each conversation costs~$0.002. Sustainable.

What's Next for MindEase

This is version 1.0, but there's so much more I want to build:

Voice Interface: Some people find it easier to talk than type. Speech-to-text input and text-to-speech responses would expand accessibility.

Multi-language Support: Nigeria has 500+ languages. Supporting Hausa, Yoruba, and Igbo would make MindEase accessible to millions more people.

Sentiment Tracking: Visualize mood over time. Show users: "You've been feeling better this week compared to last month."

Offline Support: Make it a Progressive Web App (PWA) that queues messages when you're offline and syncs when you reconnect.

Professional Handoff: If someone needs more help than I can provide, facilitate a warm handoff to a real therapist—anonymous referral with context.

Group Support: Anonymous support groups for specific issues (anxiety, grief, exam stress) with AI moderation.

Reflections: What I Learned

Building MindEase taught me that empathy can be engineered. Not perfectly, not as well as a human, but meaningfully.

The hardest part wasn't the code—it was understanding what people need when they're vulnerable. That required research, testing, iteration, and a lot of empathy.

I learned that:

  • Prompt engineering is an art. The difference between a mediocre chatbot and a great one is in the system prompt.
  • Speed shapes experience. Streaming makes the AI feel alive, even though it's just buffered text.
  • Privacy enables vulnerability. Anonymity isn't just a feature—it's the foundation of trust.
  • Cultural context matters. A globally generic therapist bot feels off. A Nigerian-tuned one feels like home.

Most importantly: technology can lower barriers. MindEase won't replace therapists, but it might help someone take the first step toward help. And that's worth building.

Final Thoughts

MindEase started as a project idea and became something I genuinely care about. Every design decision—from anonymous accounts to streaming responses to crisis detection—was driven by one question: "Would this help someone who's struggling right now?"

I don't know if MindEase will reach thousands of people or just a handful. But if even one person opens the app at 2 AM feeling alone and finds comfort in a conversation, I'll consider it a success.

Because mental health support shouldn't be a privilege. It should be as accessible as opening an app and typing "I need someone to talk to."
